PRP hair loss treatment

PRP hair loss treatment Lahore has become an effective and promising modality of treatment for androgenic alopecia with other modalities of treatment available. Androgenic alopecia is a non-scarring form of diffuse baldness having a specific pattern, with underlying genetic, environmental factors and hormonal disturbances. It is usually physiological and baldness becomes more prominent with aging. Hair growth is dependent on multiple factors and the most important one is the male hormone testosterone that is converted to 5- dihydrotestosterone in the roots of the follicles by perifollicular 5-alpha reductase. In genetically predisposed individuals, due to the effect of 5-dihydrotestosterone follicles at the top and front of the scalp, i.e., androgen-dependent areas of the scalp, become more and more fine as the years pass. Eventually with time the growth gets limited and the hair completely disappears. In women, however, the front hairline is usually not affected and baldness in females has a separate classification. Different treatment options that are available for androgenic baldness include Minoxidil solution 2%, Finasteride, Spironolactone, nutritional supplementation, low-level light therapy and hair transplantation. These therapies require a long term and continuous treatment and desirable improvement in growth are usually not observed. Still many patients have visible improvement after getting few sessions of PRP hair loss treatment in Lahore Pakistan.

Basic technique Platelet rich plasma

Platelet rich plasma hair loss treatment is minimally invasive, without any major side effects and is cost effective as compared to hair transplantation surgery. There is minimum risk of infection and immune mediated rejection following the procedure. It utilizes platelet rich plasma containing certain growth factors that play a key role in hair regrowth. Activated platelets release a large number of growth factors and cytokines from the alpha granules present inside the platelet cytoplasm during the process of wound healing. The platelet rich plasma when injected in the follicles, the injected platelets become activated and release growth factors that contribute to hair growth. The growth factors that are released play a major role in collagen synthesis, extracellular substance formation, fibroblast activation and endogenous growth factors expression. The growth factors that play role in active growth are platelet derived growth factor (PDGF), epidermal growth factor (EGF), insulin like growth factor (IGF-1), vascular endothelial growth factor (VEGF) and transforming growth factor beta (TGF-B). All these factors released from the activated platelets help in various processes that promote cell growth including chemotaxis, cellular proliferation, differentiation and angiogenesis. In addition to alpha granules, platelets also contain dense granules that contain histamine, serotonin, adenosine, dopamine and calcium. These substances increase membranous permeability and modulate the inflammatory process. However, the exact mechanism has still not been completely understood. Activated platelets may also cause induction of dermal papilla cells proliferation by activating certain signaling pathways. The activation of these pathways eventually results in increased gene transcription involved in cellular growth, differentiation and proliferation. This eventually causes hair lengthening while preventing apoptosis.

Platelet rich plasma used in platelet rich plasma hair loss treatment can be classified using various systems. Mishra’s classification for platelet rich plasma is dependent on presence or absence of white blood cells, activation status and platelet concentration coefficient in the plasma compared with whole blood. Another classification system based on presence of components like platelets and leukocytes and presence or absence of fibrin. Platelet rich plasma may also be classified as pure platelet rich plasma (P-PRP), leukocyte and platelet rich plasma (L-PRP), pure platelet rich fibrin (P-PRF) and leukocyte and platelet rich fibrin (L-PRF).

PRP component

For the preparation of platelet rich plasma hair loss treatment in Lahore, various methods are used. PRP is produced using cell separation centrifugation techniques and then is injected into the bald areas of the scalp that are androgen dependent. Some methods may have a device to reduce the number of leukocytes and increase the number of pure platelets. Despite the difference in the separation technique that is used for different methods, the basic process remains the same. Blood sample is collected in a vacutainer with anticoagulant like sodium citrate that prevents formation of blood clot and eventual activation of platelets in the drawn blood sample. Whole blood is processed through centrifugation to isolate the red blood cells. This is followed by another centrifugation to concentrate the platelets. Exogenous platelet activators like thrombin and calcium chloride might be added just before injecting the platelet rich plasma in the scalp as it results in immediate release of growth factors that are dose dependent. Non activating or resting platelet rich plasma might be injected with spontaneous activation of platelets due to exposure to dermal collagen and thrombin. Growth factor release from alpha granules of platelets starts within 10 minutes of activation of platelets with more than 95% growth factors released within one hour and it continues for almost seven days. PRP is injected intradermally in the bald areas of the scalp.

Platelet concentration is the most important factor that defines the efficacy of PRP hair loss treatment in Lahore. Studies suggest that a platelet concentration of two to six times the basal platelet count is usually needed for optimal treatment results. In addition, too much platelets might also decrease the angiogenic potential so care should be taken regarding the platelet concentration in PRP. The presence of leukocytes in the PRP might help in resistance from infection, increase release of growth factor and promote angiogenesis and cellular proliferation. While some other studies suggest that presence of neutrophils in PRP might cause an increase in the inflammatory process and contribute to destruction of surrounding tissues, caused by the release of reactive oxygen species and various proinflammatory cytokines. Red blood cells are removed in most PRP preparations. So, presence of different cells like red blood cells, white blood cells and platelets, affect the properties of platelet rich plasma and its response when it is injected into the scalp.

Adjunct Therapy

Platelet rich plasma hair treatment is used as an adjuvant along with use of other topical agents and oral medications including minoxidil, spironolactone and finasteride. The use of these topical and oral agents helps in suppressing the hormonal element of androgenic alopecia that is usually not controlled by platelet rich plasma, as it has no effect on hormonal elements affecting the hair growth. Platelet rich plasma injected intradermally is recommended as it is more effective and overall less painful. It also results in a more diffuse distribution of platelet rich plasma, thus requiring fewer injections. Injections are usually applied in the scalp in the region of hair thinning, along the hairline, vertex and crown of scalp. Treatment usually includes sessions each month for the first three months, then every three months for the first year. So, treatment with platelet rich plasma requires a total of six sessions in the first year at 1,2,3,6,9 and 12 months. Treatment results in increased hair density and better quality of life for the patient. In conclusion, the use of platelet rich plasma for androgenic alopecia is an effective treatment modality available that significantly improves the quality of life of patients with androgenic baldness. The preparation, dosage, technique of injections number and interval of injections with platelet rich plasma vary with different procedures due to lack of standardization of platelet rich plasma preparations. So, the end results of treatment are difficult to interpret and long term follow ups to determine the long-term efficacy of the treatment.

What are the benefits of stem cells over platelet rich plasma (PRP) for hair growth/restoration?

Stem cells have the ability for the proliferation and reproducibility of the hair. Normally there are three sources to take these cells 1- Fat 2- hair follicles 3- Mesenchymal cells from human placenta and available commercially in the market by the name of ” Exosomes” in injection form.
The first two methods are invasive and no guarantee for any hair regrowth and procedures are also costly. The third technique, Exosomes may facilitate cancer in the human body so this is the reason FDA did not approve it.
PRP is prepared from the same person’s blood and does not have serious side effects and is safe. However, PRP can not regenerate or regrow hair and it can only control hair fall temporarily. PRP is 60% effective in controlling hair fall for 4 to 8 weeks and 40% is failure rate. So those who do not get any benefit in the first or second session of PRP should avoid this treatment.
There is a new and emerging technique called ” Stem cells activation naturally” where inactive stem cells at the base or root of the hair are stimulated and these start functioning and become active. As a result one can get normal and thick hair in 6 to 8 months duration and we can now say that reversal of baldness in Lahore is possible at our clinic through this technique.
